Kindermusik Northwest:: Don't leave childhood without it!

1.	Please print the registration form and fill in completely. Registrations will be accepted by mail only for the month of June. Beginning July 1^st, we will also accept registrations by phone, 847-304-8947 and fax, 847-304-8963 Registrations are processed by postmark not date of receipt. We believe this is the fairest way for everyone. Although we do our best to accommodate your requests, it is not possible for everyone to get their first choice. Therefore, please list as many alternate choices as you can.
2.	Include a $75 dollar non-refundable deposit for each registered class. Your registration cannot be processed without it. Checks can be made payable to Kindermusik Northwest. If we cannot accommodate your choices, your deposit will be returned.
3.	Students must meet age requirements for each level. It is in the best interest of your child to be in an age-appropriate environment.
4.	Kindergarten parents please register for both an am and pm time per choice. We will double book your child until their kindergarten schedule is known and ask for your flexibility and understanding in order for this to work. Example: lmycI1015 / lmycI1230
5.	Please check a materials choice box. Full set of materials Sibling kit (Which is ordered when you have 2 children taking the same class or if you’ve had an older child previously enrolled in the same curricula and you want a partial set for a sibling now taking the class). Our Time Sib Kit includes: Family Activity Book, Instrument and Carrying Case. Imagine That! Sib Kit includes: Family Activity Book, Instrument and Play set. Young Child I and II Sib Kit includes: Folder and all consumable materials.
6.	The entire price of a class is Materials + Tuition. If you are registered for more than one class you will receive a $10 discount on each additional class. Please notice at the very bottom of your registration form that there are two options to choose from if you are paying by credit card. Please check only one of those boxes.
