1.	New Family Registration begins Monday, December 7th! For most accurate class availability, please phone in your registration to 847-304-8947 or fax to 847-304-8963.
2.	Include a $75 dollar non-refundable deposit for each registered class. Your registration cannot be processed without it. Checks can be made payable to Kindermusik Northwest. If we cannot accommodate your choices, your deposit will be returned.
3.	Students must meet age requirements for each level. It is in the best interest of your child to be in an age-appropriate environment.
4.	Please check a materials choice box. Full set of materials Sibling kit (Which is ordered when you have 2 children taking the same class or if you’ve had an older child previously enrolled in the same curricula and you want a partial set for a sibling now taking the class). Village Sib Kit includes Bach and Baby CD and an instrument Our Time Sib Kit includes: Family Activity Book, Instrument and Carrying Case. Imagine That! Sib Kit includes: Family Activity Book, Instrument and Play set. Young Child I and II Sib Kit includes: Folder and all consumable materials. No Materials - This is only an option if you have the full set of materials from an older child.
5.	The entire price of a class is Materials + Tuition. If you are registered for more than one class you will receive a $10 discount on each additional class. Please notice at the very bottom of your registration form that there are two options to choose from if you are paying by credit card. Please check only one of those boxes.
